New Patient Forms

Im making a site for a heart surgeon. I intend to add forms pdf that patients can fill in before visit. What is the best way to do this? Is there a way that they can fill in the forms online … or do they have too have correct software their end?

Have a look at Formsnap. Not a PDF, but seems to have the functionality you’re after, and can save to Google Sheets and/or MySQL.

1 Like which is another 3rd party form maker has the option to NOT store responses on their server - I think that is partly for HIPAA compliance.

It should be noted that Machform is not a RapidWeaver solution.

Jotforms, Formstack and Kiteworks all claim HIPAA compliance. Again these are not RW solutions but should work perfectly well. Will likely require a subscription.
